Submitting an Application
Fill out and print your application with an online printable form or a fillable PDF . Customers using screen reader software should use the online printable form. Applicants must complete Part A and Part B of the application. An authorized health care professional must complete and stamp Part C of the application. Incomplete forms will not be accepted.
Please note that the TTC will not reimburse any costs incurred to complete this application.

Applications can be submitted:
Mail completed application to the TTC with two (2) colour passport photos signed on the reverse by the authorized regulated health care professional who completes Part C of the Application. Applications can be mailed to:

Submit your application and get your photo taken at Sherbourne Station. Please note that Sherbourne Station is not accessible.
Customers can also get their photo taken at the Davisville Customer Service Centre. Please note that applications processed at the Customer Service Centre will be mailed to the applicant.
